Client Services Officer, Choice Health and Wellbeing Group. Wanneroo, Perth WA. Aged & Disability Support (Community Services & Development). Full time. $60,000 – $70,000 per year.

Position Overview: As the Client Services Officer, your role will have a variety of responsibilities, including customer retention—assisting with onboarding new customers and ensuring existing customers are getting the best service possible using our resources. Be the customers' main point of contact from local area coordinators, support coordinators, and medical practitioners to internal/external stakeholders to assist our participants in implementing their NDIS plan. As a strong communicator, you’ll build upon existing relationships and source new relationships to promote our services and create potential future opportunities for the company.
Key Responsibilities: Manage and coordinate the intake of participants. Manage record keeping, answering phone enquiries, setting up intake appointments, and conducting meetings with participants and their families. Undertake initial intake assessment, risk assessment, client matching, and organize a house visit with the participant and stakeholders to inspect the house and meet the current residents
Provide relevant information and updates to stakeholders, ensuring transparency and understanding.
Build and maintain positive relationships with stakeholders, participants, and Services. Maintain positive relationships with participants through regular communication and meetings to ensure quality care and support are provided.
Ensure participant files are compliant with NDIS regulations and quality standards. Complete accurate and thorough reports that capture participants' progress and challenges effectively. Document and report on interactions, activities, and issues as needed. Develop and manage participant service schedules, including service planning, confirmation, and cancellations. Manage participant medication and behaviour reporting procedures. Be available on-call when required, providing timely support and guidance to the team and participants during critical situations. Complete initial support plans and shift plans based on participants' goals and support requirements. Ensure participants are supported to identify, choose, and manage their own daily and lifestyle routines. Maintain effective positive relationships with internal and external stakeholders.
Attend inter-agency meetings and promote CHWG services and seek leads and referrals. Attended Expos and provider networking events and built leads for referrals. Work with the team and assist with Service Delivery as required.
Selection Criteria: Essential. Bachelor’s degree or Diploma in a relevant field in Nursing/disability/mental health/community service or equivalent experience. Demonstrated experience in engaging service delivery with disability services, including experience in referral intake and assessments.
Sound oral and written communication and customer service skills, where you will have past experience liaising with key stakeholders, including families and multi-disciplinary teams. Demonstrated ability to prioritise tasks, share information and manage your time to an excellent standard. Demonstrated strong leadership skills and ability to foster committed and collaborative teams. Demonstrated experience in supervising teams with minimal supervision. Demonstrated working knowledge of the National Disability Insurance Scheme (NDIS) and NDIS Quality & Safeguards Commission. Strong organizational and problem-solving abilities.
The successful applicant requirement: NDIS Workers Screening Check. Current Australian full driver’s license. Working With Children Check. National Police Check (within last 3 months). NDIS Worker Orientation Certificate. First Aid & CPR Certificate. Mental Health First Aid Certificate—working toward it. 2 employment references—from current and most recent employers. Available to work Monday-Friday office hours (except public holidays) and occasional weekend/Expo attendance. Willingness to sign a confidentiality agreement and conflict of interest declaration.
