We are seeking an experienced support coordinator or community services professional to join us in our growing support coordination team. The applicant must hold an appropriate bachelor's level qualification and live locally in Mandurah to be considered. Flexible working arrangements are promoted!
About the role: Reporting to the Perth Business Unit Manager, the primary objective role is to provide high-quality support coordination for NDIS participants in the South-Perth region, including Cannington, Midland, and Willeton. You will be required to do some face-to-face meetings, undertake case conferences, help to oversee and manage their NDIS budgets, and help your participants successfully implement their NDIS plan. In their direct client work, the support coordinator will focus on strengthening a participant's ability to navigate intersecting service systems, including health, justice, mental health, housing, and other social services.
To be successful in this role, you must have a Bachelor's qualification in Community Services, Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology, or other Allied Health qualification. Previous experience supporting participants that have mental health, psychosocial, or disability support needs in a case management style role. Knowledge of disability, mainstream, and community services.

Advantageous Skills and Experience: A self-starter, self-motivated person who can work independently. Previous Support Coordination Experience. Demonstrated knowledge of psychosocial diagnoses, both chronic and acute, or the ability to source accurate and critical patient information.
Solid background and knowledge of NDIS. Excellent verbal communication skills and a demonstrated ability to engage with participants and their families. Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence, and other documents.
