About the role: The state-wide better at Home initiative has been implemented to deliver more healthcare within the home. Part of this initiative is to enable patients to access Hospital in the Home (HITH) and District Nursing Services.

South West Healthcare (SWH) is seeking a motivated and skilled Registered Nurse to join the Hospital in the Home (HITH) team. As a valued member of our multidisciplinary team, the Hospital in the Home Program Coordinator will be responsible for the ongoing development of the program and provision of clinical care throughout our community.

Our teams work using systematic mechanisms that ensure regular communication and the flow of information with other clinical staff and professionals, families, carers and stakeholder groups located both internally and externally to SWH.

Position overview: Part-time (28 hours per week), On-going position. Warrnambool. HITH/PAC Nurse Level 2. Salary: $107,962.40 pro rata, plus superannuation. What you bring. The successful applicant will be an experienced and qualified Registered Nurse with the ability to demonstrate excellent skills and experience gained through a broad range of clinical services, including a proven ability to work within a team-based multi-disciplinary model.

Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communication skills are also essential. A comprehensive understanding and knowledge of legislation, regulation and contemporary practice is a must.

Applicants must hold a Bachelor of Nursing or equivalent qualifications and register with the Australian Health Practitioner Regulation Agency with no conditions and have at least 5 years' experience as a Registered Nurse.
