Full time. $84,006–$90,363 per year. The purpose of this role is to provide robust support and coordination activities for NDIS participants. As a robust support coordinator, you will work within a team environment to deliver high-quality transition support for children and adults living with varying circumstances and levels of complexity. You will match individuals with community-based services and mainstream programs and will provide information, advocacy, and referral support to implement the funded services outlined in the participants NDIS plans.
This position requires a worker who has significant experience regarding the NDIS and is able to provide complex case management across, including but not limited to: justice interface, alcohol and drugs, homelessness and mental health.
Main duties and responsibilities include but not limited to, the following: Support participants to understand their plan. Implementing risk assessments. Completing service assessments with the participant, based on their goals and the level of service defined in the NDIS Plan. Coordination of services as documented in the participant's NDIS Plan. Managing multidisciplinarity with key stakeholders to manage and respond to complex situations. Liaise with and connect with community and mainstream services (housing, education, and health). Providing complex case management. Onboarding of new participants. Monitor use of support coordination hours. Provide to the NDIA timely and relevant progress reports reflecting progress towards achieving plan goals and recommendations in the next plan. Referral management. Preparing service agreements. General administrative support.
Selection Criteria Essential: Tertiary qualification in a relevant discipline. Strong analytical and problem-solving skills. A sound understanding of relevant legislation and NDIS procedural guidelines. Demonstrated commitment to promoting a cooperative and cohesive team. Demonstrated capacity to work efficiently and to complete administrative tasks. Highly developed written and verbal communication and negotiation skills. High level of computer proficiency in MS Office suite.
Desirable: Experience in the disability, community or health sector. Experience using Datanova FlowLogic software means that members of the team will be able to assist.
