Wellfort Board of Directors is currently seeking a facilitator/consultant to conduct a review of the Board and to assist with the development of a three year plan that includes

WellFort is a nonprofit, values based organization that provides health promotion and prevention programs, primary health care and works with local residents and other agencies to increase community capacity. Members of the WellFort family include Bloom Clinic, Bramalea CHC, Diabetes Education Program, Four Corners, and Health n’ Smiles. WellFort is committed to providing inclusive, accessible and sustainable programs and services to a diversely rich community through its service delivery philosophy; Open Focused Care. We are seeking individuals with a passion and commitment towards innovation and an inclusive workplace that respects and values the diversity of every individual.

Relevant details about WellFort include:

    Was incorporated in August 2008 and has a Board of Directors of nine.
    Has experienced significant growth; from one CHC to a multi-faceted organization with five core programs. 
    Will undertake two capital projects in the next three years.
    Recently completed a strategic planning process.
    Recently changed its corporate name.

Scope of the project to include:

    A review and recommendations on the model of governance.
    A review of the Board structure and its functioning (subcommittees, position descriptions, etc)
    An assessment and plan for Board development for the next three years.
    Development of a three year rolling work plan
    A succession plan
    A bylaw review

Budget and time lines

          A plan(s) ready to implement for fiscal 14/15
          $23,000 plus HST
