The Four Villages Community Health Centre has two locations in the west end of Toronto. It is funded by the Toronto Central LHIN. Four Villages’ mandate is to provide high quality, accessible primary health care to the communities within its service area, in the context of the broader social determinants of health. Primary care, clinical services and a wide range of health promotion programs, are primarily aimed at seniors, families with young children, youth and newcomers. Staff collaborate in an interdisciplinary team environment. The focus is on the client and capacity building.
We are currently recruiting a Registered Nurse with training and expertise in foot care to work with our Chiropodists and Diabetes Education Program team to provide foot care to individuals with diabetes.
RESPONSIBILITIES
Within this community-based, interprofessional model of care, the Foot Care Nurse contributes to the provision of accessible and coordinated foot care, with the focus on diabetes foot care. This includes:
assessing clients’ physical and psychosocial needs;
providing foot care within the scope of RN, including health education and chronic disease management, on-site and off-site (home visits);
maintaining a waiting list for foot care; tasks include initiating phone calls, taking client history, engaging with family members and providing both internal and external referrals as needed;
identifying resources needed to support clinical care, health promotion and health education;
participating in the development, delivery, monitoring and evaluation of programs that focus on foot health and foster self-help, mutual aid, and capacity building;
conducting outreach to community residents who require foot care;
developing partnerships and alliances to support the diabetes programs and to address the needs of diabetic clients in the community;
reporting and recording accurately and appropriately, using electronic charting;
assisting with maintenance of equipment, supplies and resources in the chiropody areas;
providing support and consultations to staff, students and volunteers;
QUALIFICATIONS
Bachelor of Science in Nursing is preferred or RN diploma;
certificate in foot care from a recognized institution;
current registration with the College of Nurses of Ontario;
three to five years nursing experience in a community setting or combination of hospital and community and/or public health settings;
at least one year of experience with foot care;
thorough knowledge and proficiency in current nursing physical assessment and foot care treatment methods;
knowledge of and experience with diabetic foot screening and assessments an asset;
demonstrated ability, commitment to and knowledge of community health care and community outreach strategies;
open-minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity;
demonstrated ability to work in an interprofessional team environment and strong interpersonal skills;
proficiency in the use of computers and various software applications;
excellent communication, decision-making, problem-solving, conflict management and time management skills;
second language, relevant to catchment area populations an asset;
Hours of work: 35 hours per week
