Case Manager - Aged Care, Coffs Harbour & North Coast, NSW Home Support Services is a dynamic and well established provider of community aged care services throughout the Coffs Harbour, Bellingen and Nambucca Local Government Areas. We are seeking applications for a full time Case Manager to manage clients across a range of programs including Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP).
The successful applicant must have: Relevant tertiary (or other) qualifications and knowledge of community aged care. Experience in the management of Govt. funded programs. including HCP and CHSP. Skilled in client assessments, goal planning and risk management. Excellent communication, negotiating skills & good time management. Police check clearance. Current NSW drivers licence. Own car with comprehensive insurance.
