Inner South East Post Acute Care, (0.6 EFT) 22.8 Hrs/Week - Fixed Term until June 2016

ISEPAC is responsible for developing and implementing care packages for residents of the Cities of Port Phillip, Stonnington, Glen Eira and Bayside who have been discharged from a publicly funded hospital setting and require additional services during the recuperative phase of their illness. This position combines assessment, referral and care coordination within both hospital and community settings to ensure the client’s needs are met.

To be suitable for the role you will ideally possess the following: Degree in relevant profession. Eligibility for membership with a relevant professional association. Experience and demonstrated ability in the provision of case coordination or provision of services to persons with complex hospital discharge needs. Commitment to, and enthusiasm for, working with the target group. Well-developed assessment and problem solving skills.
